Event Request Form

This form is designed to help you request to reserve one of our venues at New Life. Since there are many requests, we will honor them on a first-come, first-served basis. Of course church events will take precedent, but we will do our best to accommodate your needs. Charges for venues may apply and will be applied according to our rental schedule. Please request events at least 30 days prior to the event date. If you do not receive a response within 1 week from submission, please contact the church office at (479) 524-4823 to verify your request has been received.
